Immunization & Tuberculosis Screening Certificate (Non-Healthcare Professions Version)

The current 2018 – 2019 Immunization and Tuberculosis Screening Certificate will remain active for students entering Georgetown University’s summer session ONLY.  All Medical, Nursing and GEMS (health professions) should not use this form and should contact their respective departments for immunization requirements.

A new online submission process called PyraMed is being developed for the incoming Fall 2019 students and will become active on June 1, 2019.  Thereafter, the submission process for the first year students should be completed by July 15th for the Fall semester and December 15th for the Spring semester.  All Medical, Nursing and GEMS (health professions) should not use the online submission process and should contact their respective departments for immunization requirements.  The health center remains busy during the summer and fall months verifying and entering immunization data for each new student, so the sooner you submit your form the better.   For more information regarding submission of the form, please see our Frequently Asked Questions section.

Beginning in the fall of 2008, meningitis vaccination was made a requirement for incoming freshman living in residence halls. For more information about meningitis, please see the CDC Meningococcal Vaccination information. While we strongly encourage students to be obtain a meningitis vaccine, you may waive this rquirement.  If you choose to waive the vaccine requirement, please submit the Meningitis Vaccine Waiver form when you submit your immunization certificate.
