Early Summer Arrival - Specific Incoming Student Groups

Student Health Insurance 2017 - 2018 Platinum Premier Plan Year

Amendment to the Acceptance and Waiver Terms of Agreement for Select Early Summer Arrival Incoming Student Groups

Note: Early Arrival refers to specific incoming student groups arriving before the New Premier Plan Year Effective Date of August 15.

The incoming student groups identified below are required to have health insurance coverage before August 15, 2017. Therefore, in addition to being charged for the annual 2017-2018 Premier Plan for the term of coverage August 15, 2017 through August 14, 2018, they:

  • Are also charged for a prorated 2016-2017 Early Arrival summer term,
  • Have a different deadline to submit their acceptance or waiver, and
  • Have a different date in which their other coverage must be in effect for waivers they submit.

The 2017-2018 Premier Plan Acceptance or Waiver election submitted by each student in the groups listed below will also apply to the 2016-2017 Early Summer Arrival election and premium charge(s) on each student’s account.

The Terms of Agreement and conditions for the 2017-2018 Premier Plan (to be updated by July 2017) also apply to the 2016-2017 Early Summer Arrival Incoming Students in the groups described below (with the exception of the benefits detailed in the 2016-2017 Premier Plan Description of Benefits). The provisions are amended as follows:

Professional Studies (MPS-SCS) International Students:

  • Early Summer Arrival Coverage Term: May 15, 2017 through August 14, 2017
  • Additional Summer 2017 Charge for the above 2016-2017 Early Arrival Coverage Term: $674
  • Deadline for Accepting or Waiving the annual 2017-2018 Premier Plan: July 31, 2017
  • If waiving, the effective date for other coverage must be no later than July 31, 2017
  • If dependent coverage is added to the annual 2017-18 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
    • $1348 per student & spouse or student & one child
    • $2022 per student & two or more children
    • $2022 per student, spouse, and one child
    • $2696 per student, spouse and two or more children

Masters of Policy Management (MPM) and Nurse Anesthesia Students:

  • Early Summer Arrival Coverage Term: June 1, 2017 through August 14, 2017
  • Additional Summer 2017 Charge for the above 2016-2017 Early Arrival Coverage Term: $563
  • Deadline for Accepting or Waiving the annual 2017-2018 Premier Plan: July 31, 2017
  • If waiving, the effective date for other coverage must be no later than July 31, 2017
  • If dependent coverage is added to the annual 2017-18 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
    • $1126 per student & spouse or student & one child
    • $1689 per student & two or more children
    • $1689 per student, spouse, and one child
    • $2252 per student, spouse and two or more children

Biomedical Sciences, PhD Seeking, Chemistry Department, Graduate School, Community Scholars Program, GEMS Program, Law Foundations Program, International Students in Masters in Health Systems Administration, PHD Chemistry Students, Medical Students returning from a Leave of Absence, and Communication Tools for Success Students:

  • Early Summer Arrival Coverage Term: July 1, 2017 through August 14, 2017
  • Additional Fall 2017 Charge for the above 2016-2017 Early Arrival Coverage Term: $366
  • Deadline for Accepting or Waiving the annual 2017-2018 Premier Plan: July 31, 2017
  • If waiving, the effective date for other coverage must be no later than July 31, 2017
  • If dependent coverage is added to the annual 2017-18 coverage, dependent coverage must also be added to the Early Summer Arrival coverage, accordingly, as follows:
    • $ 732 per student & spouse or student & one child
    • $1098 per student & two or more children
    • $1098 per student, spouse, and one child
    • $1464 per student, spouse and two or more children

MBA Incoming Students:

  • Early Summer Arrival Coverage Term: July 15, 2017 through August 14, 2017
  • Additional Fall 2017 Charge for the above 2016-2017 Early Arrival Coverage Term: $275
  • Deadline for Accepting or Waiving the annual 2017-2018 Premier Plan: August 31, 2017
  • If waiving, the effective date for other coverage must be no later than August 31, 2017
  • If dependent coverage is added to the annual 2017-18 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
    • ​$  550 per student & spouse or student & one child
    • $  825 per student & two or more children
    • $  825 per student, spouse, and one child
    • $1100 per student, spouse and two or more children

Medical First Year Incoming Students:

  • Early Summer Arrival Coverage Term: August 1, 2017 through August 14, 2017
  • Additional Fall 2017 Charge for the above 2016-2017 Early Arrival Coverage Term: $164
  • Deadline for Accepting or Waiving the annual 2017-2018 Premier Plan: August 31, 2017
  • If waiving, the effective date for other coverage must be no later than August 31, 2017
  • If dependent coverage is added to the annual 2017-18 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
