Early Summer Arrival - Specific Incoming Student Groups

Student Health Insurance 2018 - 2019 Premier Plan Year

Amendment to the Acceptance and Waiver Terms of Agreement for Select Early Summer Arrival Incoming Student Groups

Note: Early Arrival refers to specific incoming student groups arriving before the New Premier Plan Year Effective Date of August 15.

The incoming student groups identified below are required to have health insurance coverage before August 15, 2018. Therefore, in addition to being charged for the annual 2018-2019 Premier Plan for the term of coverage August 15, 2018 through August 14, 2019, they:

  • Are also charged for a prorated 2017-2018 Early Arrival summer term,
  • Have a different deadline to submit their election survey, and
  • Have a different date in which their other coverage must be in effect for waivers.

The 2018-2019 Premier Plan Acceptance or Waiver election submitted by each student in the groups listed below will also apply to the 2017-2018 Early Summer Arrival election and premium charge(s) on each student’s account.

The Terms of Agreement and conditions for the 2018-2019 Premier Plan (to be updated by July 2018) also apply to the 2017-2018 Early Summer Arrival Incoming Students in the groups described below (with the exception of the benefits detailed in the 2017-2018 Premier Plan Description of Benefits). The provisions are amended as follows:

Professional Studies (MPS-SCS) International Students:

  • Early Summer Arrival Coverage Term: May 15, 2018 through August 14, 2018
  • Additional Summer 2018 Charge for the above 2017-2018 Early Arrival Coverage Term: $720
  • Deadline for Accepting or Waiving the annual 2018-2019 Premier Plan: July 31, 2018
  • If waiving, the effective date for other coverage must be no later than July 31, 2018
  • If dependent coverage is added to the annual 2018-19 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
    • $1440 per student & spouse or student & one child
    • $2160 per student & two or more children
    • $2160 per student, spouse, and one child
    • $2880 per student, spouse and two or more children

Masters of Policy Management (MPM) and Nurse Anesthesia Students:

  • Early Summer Arrival Coverage Term: June 1, 2018 through August 14, 2018
  • Additional Summer 2018 Charge for the above 2017-2018 Early Arrival Coverage Term: $598
  • Deadline for Accepting or Waiving the annual 2018-2019 Premier Plan: July 31, 2018
  • If waiving, the effective date for other coverage must be no later than July 31, 2018
  • If dependent coverage is added to the annual 2018-19 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
    • $1196 per student & spouse or student & one child
    • $1794 per student & two or more children
    • $1794 per student, spouse, and one child
    • $2392 per student, spouse and two or more children

Biomedical Sciences, PhD Seeking, Chemistry Department, Graduate School, Community Scholars Program, GEMS Program, Law Foundations Program, International Students in Masters in Health Systems Administration, PHD Chemistry Students, Medical Students returning from a Leave of Absence, and Communication Tools for Success Students:

  • Early Summer Arrival Coverage Term: July 1, 2018 through August 14, 2018
  • Additional Fall 2018 Charge for the above 2017-2018 Early Arrival Coverage Term: $382
  • Deadline for Accepting or Waiving the annual 2018-2019 Premier Plan: July 31, 2018
  • If waiving, the effective date for other coverage must be no later than July 31, 2018
  • If dependent coverage is added to the annual 2018-19 coverage, dependent coverage must also be added to the Early Summer Arrival coverage, accordingly, as follows:
    • $ 764 per student & spouse or student & one child
    • $1146 per student & two or more children
    • $1146 per student, spouse, and one child
    • $1528 per student, spouse and two or more children

MBA Incoming Students:

  • Early Summer Arrival Coverage Term: July 15, 2018 through August 14, 2018
  • Additional Fall 2018 Charge for the above 2017-2018 Early Arrival Coverage Term: $281
  • Deadline for Accepting or Waiving the annual 2018-2019 Premier Plan: August 31, 2018
  • If waiving, the effective date for other coverage must be no later than August 31, 2018
  • If dependent coverage is added to the annual 2018-19 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:

    • $  843 per student & two or more children
    • $  843 per student, spouse, and one child
    • $1124 per student, spouse and two or more children

Medical First Year Incoming Students and Incoming Special Master's Program, Physiology

  • Early Summer Arrival Coverage Term: August 1, 2018 through August 14, 2018
  • Additional Fall 2018 Charge for the above 2017-2018 Early Arrival Coverage Term: $159
  • Deadline for Accepting or Waiving the annual 2018-2019 Premier Plan: August 31, 2018
  • If waiving, the effective date for other coverage must be no later than August 31, 2018
  • If dependent coverage is added to the annual 2018-19 coverage, dependent coverage must also be added to the Early Arrival coverage, accordingly, as follows:
