The patient Release of Information Form allows CAPS to discuss your situation with third parties if you would like us to do so.  See the Sample Form for guidance as to how to fill out the form.

The Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information.

Medical Leave of Absence

The Community Provider Report Form will be needed when applying to return from Medical Leave of Absence.

The Release of Information Form  should also be completed when applying to return from Medical Leave of Absence so that CAPS may communicate with any necessary third parties (e.g., hometown providers, Academic Deans, parents, etc.). See the Sample Form for guidance as to how to fill out the form.
